          【摘要】 目的 系统评价痤疮瘢痕 CO2 点阵激光术后应用碱性成纤维细胞生长因子 ( bFGF) 治疗局部创面的临床疗效。方法 检索 Cochrane Library、中国学术期刊数据库 (万方) 等数据库建库至 2023 年 8 月公开发表的痤疮瘢痕 CO2 点阵激光术后应用 bFGF 治疗局部创面的中英文文献, 经筛选后对符合要求的文献进行质量评价与数据分析。结果 共纳入文献15篇, 包含1 266例痤疮瘢痕患者。Meta分析结果显示, 与单纯应用 CO2 点阵激光照射相比, 联合应用bFGF治疗的患者总有效率明显更高 ( OR = 4.15, 95% CI 为 2.63 ~ 6.56, P < 0.00001)、红斑及结痂持续时间更短 ( SMD = - 1.53、- 1.85, 95% CI 为- 2.38 ~ - 0.68、- 2.67 ~- 1.03, P = 0.0004、P<0.00001)、痤疮瘢痕权重 (ECCA) 评分及不良反应发生率更低 ( SMD/ OR = -1.96、0.31, 95%CI 为 -2.85~ -1.07、0.20~ 0.50, P均<0.00001)。结论 与单纯应用 CO2 点阵激光相比, 在其基础上联合应用 bFGF治疗痤疮瘢痕, 更能有效改善瘢痕症状, 缩短激光术后红斑及结痂持续时间, 降低不良反应发生率, 疗效更显著。

          【Abstract】 Objective To systematically evaluate the clinical efficacy of basic fibroblast growth factor (bFGF) in treating local wounds of acne scars after CO2 fractional laser operation. Methods Chinese and English literature search was conducted to retrieve studies on bFGF in treating local wounds of acne scars after CO2 fractional laser operation published from the establishment of the databases to August 2023, from databases including Cochrane Library and China Science Peri-odical Database (Wanfang). After screening the retrieved literature, quality assessment and data analysis were performed on the finally included studies. Results A total of 15 papers screened out from the retrieved literature were included in the study, with 1 266 acne scar patients involved. Meta?analysis showed that, compared with the application of CO2 fractional laser alone, patients treated with the combined application of bFGF had a higher total effective rate after treatment (OR = 4.15, 95%CI: 2.63-6.56, P<0.00001), shorter duration of erythema and crusting (SMD= -1.53 and -1.85, 95%CI:-2.38 - -0.68, -2.67 - -1.03, P = 0.0004, P<0.00001), lower echelle d’evaluation clinique des cicatrices d’acne (ECCA) score and incidence of adverse reactions (SMD/ OR= -1.96 and 0.31, 95%CI: -2.85 - -1.07, 0.20 - 0.50, both P<0.00001). Conclusion Compared with the application of CO2 fractional laser alone, the treatment combined with bFGF is more effective in improving scar symptoms, reducing the erythema and crusting duration and the incidence of ad verse reactions, and the clinical efficacy is more definite.
