【摘要】 目的 分析探讨中药浸浴在四肢烧伤残余创面中的应用效果。方法 选取2018 年 5 月至 2022 年 9月山东第一医科大学第二附属医院收治的48例四肢烧伤残余创面患者作为研究对象, 按照不同治疗方法将其分为试验组(24例) 和对照组(24例), 试验组患者局部创面于中药浸浴后涂抹牛碱性成纤维细胞生长因子凝胶及莫匹罗星软膏治疗,对照组患者局部创面仅予以牛碱性成纤维细胞生长因子凝胶及莫匹罗星软膏治疗, 对比观察两组患者创面渗出情况、血清炎性因子水平、疼痛程度、创面病原菌阳性率及上皮化率。结果 治疗 14 d 时,试验组患者创面渗出明显少于对照组(t = 3.349, P = 0.002)、降钙素原(PCT)及C-反应蛋白 (CRP) 水平明显低于对照组(t = 8.702、12.580, P均<0.001)、疼痛程度明显轻于对照组 (Z = -2.893, P = 0.004); 治疗7、14 d时, 试验组患者创面病原菌阳性率均明显低于对照组(χ2 = 4.148、7.111, P = 0.042、0.008)、创面上皮化率均明显高于对照组 (t = 12.441、63.142, P均<0.001)。结论 给予四肢烧伤残余创面于中药浸浴后涂抹牛碱性成纤维细胞生长因子凝胶及莫匹罗星软膏治疗, 能够有效减轻创面渗出及炎症反应, 抑制病原菌生长繁殖, 降低疼痛程度, 提高上皮化率。 |

【Abstract】 Objective To analyze the clinical efficacy of Chinese medicine immersion bathing in the treatment of residual burn wounds on extremities. Methods 48 patients with residual burn wounds on extremities admitted to The Second Affiliated Hospital of Shandong First Medical University from May 2018 to September 2022 were enrolled as the research subjects and divided into the experimental group (n = 24) and the control group (n = 24) according to the different treatment methods. The local wounds of patients in the experimental group were treated with the external application of bovine basic fibroblast growth factor gel and mupirocin ointment after Chinese medicine immersion bathing, whereas the local wounds of patients in the control group were treated with the external application of bovine basic fibroblast growth factor gel and mupirocin ointment alone. Wound exudation, serum inflammatory factor level, pain degree, wound pathogen positivity rate, and epithelialization rate were compared between the two groups. Results On day 14 of treatment, the volume of wound exudate in the experimental group was significantly less than that in the control group (t = 3.349, P= 0.002). Levels of procalcitonin (PCT) and C-reactive protein (CRP) in the experimental group were significantly lower than those in the control group ( t = 8.702 and 12.580, both P< 0.001). Pain degree of patients in the experimental group was obviously milder than that in the control group (Z= -2.893, P= 0.004). On day 7 and 14 of treatment, the wound pathogen positivity rates of patients in the experimental group were significantly lower than those in the control group (χ2 = 4.148 and 7.111,P= 0.042 and 0.008), and the wound epithelialization rates of patients in the experimental group were significantly higher than those in the control group (t = 12.441 and 63.142, both P<0.001). Conclusion The external application of bovine basic fibroblast growth factor gel and mupirocin ointment after Chinese medicine immersion bathing in the treatment of residual burn wounds on extremities can effectively reduce wound exudation and inflammatory reaction, inhibit the growth and reproduction of pathogen, reduce the degree of pain, and increase epithelialization rate. |
