          【摘要】 目的 探讨分析龟鹿二仙胶在创伤性脊柱骨盆分离术后的应用效果。 方法 选取 2019 年 1 月至2021 年 11 月信阳一五四医院收治的 70 例创伤性脊柱骨盆分离患者作为研究对象, 按照不同治疗方法将其分为研究组(35例) 和对照组 (35例), 对照组患者经皮骶髂螺钉内固定术后行抗感染、营养支持、预防下肢深静脉血栓形成等常规治疗, 研究组患者经皮骶髂螺钉内固定术后在常规治疗的基础上联合应用龟鹿二仙胶治疗, 对比观察两组患者骨盆功能、临床疗效、骨代谢指标水平以及不良事件发生情况。结果 治疗 6 个月后, 研究组患者Majeed评分中疼痛、工作、坐、站评分均明显高于对照组 ( t = 2.633、 3.089、 4.408、 2.594, P = 0.011、P =0.003、P<0.001、P = 0.012); 研究组患者治疗总有效率为 94.29%, 明显高于对照组患者的治疗总有效率77.14% (χ2 = 4.200, P= 0.040); 研究组患者血清骨钙素 (OCN)、骨保护素 (OPG)、Ⅰ型前胶原氨基端前肽(PⅠNP)、 碱性磷酸酶 (ALP) 水平均明显高于对照组(t = 4.059、2.762、7.346、2.955, P<0.001、P = 0.007、P<0.001、P= 0.004); 研究组患者不良事件发生率为 11.43%, 与对照组患者的不良事件发生率 20.00%无明显差异 (χ2 = 0.971, P= 0.324)。结论 创伤性脊柱骨盆分离术后采用龟鹿二仙胶治疗, 能够明显改善患者骨代谢水平, 促进骨盆功能恢复, 效果显著。

          【Abstract】 Objective To analyze the effect of Guilu Erxian Glue application after surgery for traumatic spinopelvic dissociation. Methods 70 patients with traumatic spinopelvic dissociation at The NO.154 Center Hospital of PLA from January 2019 to November 2021, were enrolled as research subjects, and divided, based on the different treatments they received, into study group (n = 35) and control group (n = 35). After percutaneous sacroiliac screw fixation, patients in the control group were managed with routine treatments such as fighting infection, providing nutritional support and preventing formation of lower extremity deep vein thrombosis, while patients in the study group were given the application of Guilu Erxian Glue besides the same routine treatments as in the control group. The pelvic function, clinical efficacy, bone metabolism indexes and occurrence of adverse reactions were compared between the two groups. Results After 6 months of treatment, the Majeed scores of pain, work, sitting and standing in the study group were all significantly higher than those in the control group (t = 2.633, 3.089, 4.408 and 2.594, P= 0.011, P= 0.003, P<0.001, P= 0.012). The total effective rate was 94.29% in the study group, being remarkably higher than 77.14% in the control group (χ2 = 4.200, P= 0.040). The serum levels of osteocalcin (OCN), osteoprotegerin (OPG), procollagen typeⅠN-terminal propeptide ( P ⅠNP) and alkaline phosphatase (ALP) in the study group were all significantly higher than those in the control group ( t = 4.059, 2.762, 7.346 and 2.955, P<0.001, P= 0.007, P<0.001, P= 0.004). The incidence of adverse reactions of patients was 11.43% in the study group, which showed no significant difference compared with the corresponding 20.00% in the control group (χ2 = 0.971, P= 0.324). Conclusion The application of Guilu Erxian Glue after the percutaneous sacroiliac screw fixation for traumatic spinopelvic dissociation can significantly enhance the patients’ bone metabolism and promote the recovery of pelvic function, presenting remarkable therapeutic effect.
